【摘要】 目的 探讨经皮淋巴管造影术在外科术后乳糜漏诊治中的应用。方法 回顾性分析2019年7月至2022年3月四川省人民医院收治的10例经低脂中链三酰甘油饮食、全胃肠外营养、负压引流、局部加压包扎或生长抑素药物等保守治疗无效的外科术后乳糜漏患者临床资料。10例患者中乳糜漏发生于甲状腺癌术后4例，腹腔占位术后、肺癌切除术后、肾癌切除术后、胸部骨折术后、淋巴管囊肿切除术后、卵巢癌切除术后各1例，均接受淋巴管造影术诊治。结果 10例患者淋巴管造影术均获成功，碘油平均用量为9.5 mL（6～15 mL），无手术相关并发症发生。术中观察到碘油渗漏7例，仅经由淋巴管造影术停止乳糜漏7例，联合淋巴管结扎、胸导管结扎、硬化治疗停止乳糜漏各1例。平均随访14个月（4～36个月），9例患者无复发。结论 淋巴管造影术不仅可诊断和鉴别保守治疗效果不佳的外科术后乳糜漏患者渗漏部位，而且能安全有效地治疗渗漏部位。
