【摘要】 目的 探讨3D-DSA双容积重建技术辅助经皮穿刺三叉神经节微球囊压迫术(PBC)治疗原发性三叉神经痛的临床效果。方法 回顾性分析2019年6月至2020年7月采用3D-DSA双容积重建技术辅助PBC治疗的40例三叉神经痛患者的临床资料。患者均行颅底Dyna-CT三维重建,并辅助引导C臂透视下穿刺卵圆孔。抵达卵圆孔后将球囊导管置入麦克氏(Meckel)囊,注入造影剂充盈球囊压迫三叉神经半月节。相同位置再行颅底Dyna-CT三维重建,利用3D-DSA双容积重建引导调整球囊进行最优位置压迫并评估疗效。结果 术后36例疼痛症状治愈,3例明显缓解,1例无缓解,总治愈率97.5%。术中梨形显影38例,显影治愈率100%。术后出现口角疱疹3例,患侧面部麻木2例,咀嚼无力2例,均经治疗后痊愈。手术前后各时间段视觉模拟量表(VAS)和患者满意度(PSS)评分显示疗效较好(P<0.05)。结论 3D-DSA双重容积重建技术辅助PBC疗效明显,术后并发症轻微且满意度理想,具有良好的临床应用价值。
