【摘要】 介绍CT引导下行125I粒子植入腹盆部肠道周围肿瘤如何规避肠道的常用方法，为减少穿刺导致的肠道损伤提供技术指导。在腹盆部肿瘤的125I粒子植入术中，从如何做好术前准备、术中如何进行安全有效操作、术后注意事项3个方面对如何规避肠道的方法进行总结。术前做好胃肠道准备，必要时肠道造影；术中选择合适体位和穿刺路径，必要时进行肠道造影、导丝引导、制造隔离带、渐循式进针、针尖离去以及定位标记；术后及时处理并发症。其中，选择合适穿刺路径包括：经脊柱两侧、经骶尾骨旁、经肝、经椎间盘、其他（必要时选择经胃、经肠系膜间、经脾路径）。粒子植入术中灵活地联合使用本文提供的方法与技巧，可有效规避肠道，减少穿刺导致的肠道损伤。
