摘要:
【摘要】 目的 中央胆管旁肝癌被认为是热消融疗法的禁忌证,现报道一种全新的胆管冷却保护技术。方法 14例中央胆管旁肝癌患者接受了肝胆管冷却保护技术辅助下的经皮肝穿刺肝肿瘤微波消融术(PMWA)。结合文献报道及临床实践,探讨合理的标准手术流程并分析临床结果。结果 初始的14例患者顺利获得完全的微波消融,肝脏增强MRI显示均获得欧洲肝病研究学会(EASL)标准的完全缓解,未出现胆道并发症。结论 胆管冷却保护技术能够保护肝内胆管免受微波热能量损害,扩大了肝癌患者接受微波消融疗法的适应证范围,且可在肝癌微波消融术中重复使用。
