摘要:
 【摘要】 目的 对比分析彩色多普勒超声（彩超）与X线静脉造影诊断下肢静脉性溃疡中功能不全的穿静脉（IPV）的检出率和准确性。方法 2008年9月至2011年8月收治下肢静脉性溃疡患者35例，采用彩超和静脉造影检查下肢静脉，比较两种方法诊断IPV的检出率及准确性。结果 35例患者（44条患肢）共显示238条穿静脉，静脉造影检出233条，彩超检出236条（P = 1.0）；静脉造影检出IPV 100条（42.0%），彩超以反流时间0.5 s为界值，检出34条（14.4%），以0.35 s为界值，检出111条（48.0%）；穿静脉和IPV主要位于小腿下段。以静脉造影为参照，彩超3个界值（反流时间0.5 s、0.35 s及直径3 mm）中，反流时间0.5 s和0.35 s的特异度分别为100%和83.2%，敏感度分别为34%和89%，Kappa值分别为0.37和0.71；联合反流时间0.35 s和直径3 mm为界值，则特异度为95.4%，敏感度为82%， Kappa值为0.79。结论 在下肢静脉性溃疡者中，彩超诊断IPV检出率高，与X线静脉造影一致性好；彩超联合反流时间0.35 s和直径3 mm作为界值，诊断IPV准确性更高。
