摘要:
目的 探讨肾上腺静脉采样(AVS)的技术要点。 方法 对112例拟诊原发性醛固酮增多症(PA)的患者行AVS检查,记录两侧肾上腺静脉的造影形态,计算两侧插管成功率和并发症发生率。 结果 左侧肾上腺静脉以腺体型最为常见,右侧肾上腺静脉以三角型最为常见。肾上腺血样皮质醇/外周血样皮质醇比值 ≥ 3者在左侧肾上腺静脉有106例,插管成功率为94.6%;右侧有103例,插管成功率为92.0%,但两组间差异无统计学意义(P = 0.42);并发症主要是疼痛和局部血肿,发生率分别为2.7%(2/106)和6.3%(7/103)。 结论 认识肾上腺静脉的造影形态是AVS成功的前提。AVS插管成功率高,并发症少,是PA分型、定侧诊断可靠的检查方法。
