摘要:
目的 评估动静脉联合溶栓（ＩＶ／ＩＡ）与动脉溶栓（ＩＡ）治疗急性期缺血性脑卒中的疗效和风险。方法 ４６例符合溶栓适应证且无禁忌证的急性期缺血性脑卒中患者随机分组接受ＩＶ／ＩＡ（２４例）或ＩＡ（２２例）治疗， 评估不同方法治疗后血管再通率、早期临床改善率、症状性脑出血发生率、较好转归率及病死率。结果 ＩＶ／ＩＡ组从发病到接受溶栓的时间平均为２５５ ｍｉｎ，明显低于ＩＡ组的３１０ ｍｉｎ（Ｐ ＝ ０．０１２）。ＩＶ／ＩＡ组和ＩＡ组溶栓后闭塞血管再通率分别为５４．１％和４０．９％（Ｐ ＝ ０．２２６），症状性颅内出血率则为１６．７％和２２．７％（Ｐ ＝ ０．３６１）。术后３个月ＩＶ／ＩＡ组较好转归率（５４．２％）高于ＩＡ组（３６．４％），前者病死率（８．３％）低于后者（９．１％），但差异均无显著性（Ｐ ＝ ０．２２６和０．９２７）。 结论 相对于动脉溶栓，动静脉联合联合溶栓的疗效可能更佳，风险较低，但结果仍有待于大样本、双盲、对照试验证实。
