摘要:
目的 分析经子宫动脉化疗栓塞治疗宫颈癌的临床价值。方法 对病理证实的３３例子宫颈癌患者行双侧子宫动脉灌注化疗栓塞术，化疗药物选用ＤＤＰ／ＬＯＨＰ、ＥＡＤＭ、ＭＭＣ 和５Ｆｕ，栓塞物选用明胶海绵。通过治疗前后的患者临床症状和瘤体变化情况进行疗效评价。结果　　　病理证实３３例宫颈癌患者中，鳞癌２９例，腺癌４例。按照ＦＩＧＯ临床分期：Ⅰ期６例、Ⅱ期２３例、Ⅲ期４例。３３例５７支子宫动脉超选插管成功并给予栓塞化疗；９支子宫动脉未能插入者给予髂内动脉灌注化疗；３３例患者中行３次介入治疗者２例，行２次者２例，仅行１次介入治疗者２９例；首次介入治疗１周后症状缓解率鳞癌９６％，腺癌１００％。１个月后肿瘤消退近期有效率达３０．３％，其中鳞癌２７．６％，腺癌５０．０％。介入治疗后手术切除２１例，介入治疗后或手术切除后结合放疗１１例。主要不良反应为发热、白细胞下降、恶心、呕吐和腹痛。结论　　　子宫动脉化疗栓塞是宫颈癌综合治疗的重要新辅助治疗之一。
