【摘要】 目的 探讨睡眠质量等因素对接受心房颤动(房颤)导管消融术治疗患者房颤复发的影响。方法 选取2018年4月至2019年10月在东南大学附属中大医院接受导管消融术(射频消融或冷冻球囊消融)治疗成功的房颤患者,随访术后房颤复发情况。根据房颤是否复发,将患者分为复发组和未复发组,分析两组患者基本临床特征、基线疾病状态、睡眠质量等因素与房颤复发的关系。结果 共纳入102例患者,术后随访6~24个月,平均(15.27±5.29)个月。确诊房颤复发25例(24.5%),未复发77例。复发组、未复发组间左心房内径(LAD)、体质量指数(BMI)、左心室射血分数(LVEF)、房颤类型及睡眠效率等指标比较,差异均有统计学意义(P<0.05)。多因素logistic回归分析显示,睡眠效率是导管消融术后房颤复发的独立危险因素(OR=1.688,95%CI=1.034~2.753)。受试者工作特征曲线(ROC)分析显示,睡眠效率得分>0.5为预测导管消融术后房颤复发的最佳界点,曲线下面积(AUC)为0.671(95%CI=0.554~0.778),灵敏度为80%,特异度为51.9%。结论 睡眠效率是房颤导管消融术后复发的独立危险因素,睡眠效率得分>0.5具有较好的预测价值。
