摘要:
目的 探讨十二指肠乳头腺瘤内镜切除术后住院时间延长的风险因素并构建预测模型。方法 回顾性分析2019年1月至2023年12月在南京大学医学院附属鼓楼医院行十二指肠乳头腺瘤内镜切除的213例患者资料,以术后住院时间是否超过中位住院时间(7 d)分为术后住院时间正常组(<7 d,n=155)和术后住院时间延长组(≥7 d,n=58)。比较两组患者的临床资料,采用单因素及多因素logistic回归分析术后住院时间延长的独立影响因素,并构建风险预测模型。结果 多因素分析显示:ACCI(age-adjusted Charlson comorbidity index)≥4分、术中穿孔及术后病理癌变为术后住院时间延长的独立危险因素(P<0.05)。根据logistic回归分析结果构建模型的受试者工作特征(ROC)曲线下面积为0.720,95%CI:0.636～0.796,模型区分能力好,Hosmer-Lemeshow显示拟合优度良好(χ2=3.304,P=0.913)。结论 ACCI≥4分、术中穿孔及术后病理癌变是十二指肠乳头腺瘤内镜切除术后住院时间延长独立危险因素; 建立的风险预测模型,可评估患者术后住院时间延长风险,为早期临床干预提供依据。
