摘要:
目的 探讨超声引导经腋静脉完全植入式输液港植入术的学习曲线。方法 前瞻性收集2024年10至2025年11月由同一初学者连续开展的38例超声引导经腋静脉完全植入式输液港植入术的临床资料。以手术时长为观察指标,应用累积和(cumulative sum,CUSUM)分析法构建学习曲线,识别学习曲线拐点,进而将学习曲线划分为学习阶段与熟练阶段,比较两个阶段患者的基线资料、手术时长和并发症发生率等指标。结果 所有患者手术均顺利完成,手术时长随开展例数递增呈递减变化。累积和分析法学习曲线最佳拟合方程为:CUSUM=0.772+7.108x-0.196x2(R2=0.987,P<0.001),学习曲线拐点在第19例。由此可见,第1~19例属于学习阶段,第20~38例为熟练阶段。两个阶段间患者基线资料对比,差异无统计学意义(P>0.05)。熟练阶段手术时长(24.0±2.96)min,短于学习阶段的(31.47±3.66)min(P<0.05)。学习阶段患者出现的并发症有导管末端贴壁(2例)、植入物周围皮下血肿各(2例)、无菌操作不规范(5例),熟练阶段发生1例导管旋转退缩致打折堵塞,两个阶段患者均无血栓、血胸、气胸、静脉血栓、感染等严重并发症发生。结论 应用累积和分析法能够客观量化超声引导经腋静脉完全植入式输液港植入术的学习曲线; 经过约19例手术,初学者手术水平可以实现从学习阶段进入熟练阶段。超声引导经腋静脉完全植入式输液港植入术具有显著的安全性和可重复性,系统的规范化培训可使初学者顺利渡过学习阶段。
