摘要:
目的 比较CT引导下经皮穿刺微波消融与冷冻消融治疗肿瘤原发灶-淋巴结-远处转移分期ⅠA期周围型肺癌的效果和安全性。方法 回顾性分析2018年5月至2022年9月贵州航天医院收治的40例经病理确诊为TNM分期ⅠA期周围型肺癌患者临床及随访资料。根据实际实施消融技术将患者分为MWA组和CRA组。所有患者消融治疗均获成功,并完成满3年随访。采用Kaplan-Meier法分析时间-事件终点肿瘤局部控制率、无进展生存期,Log-Rank检验进行组间比较。卡方检验或Fisher精确检验比较组间消融相关并发症发生率。结果 MWA组、CRA组36个月累计局部控制率分别为80%、85%,PFS率分别为90%、85%,差异无统计学意义。MWA组、CRA组发生消融相关气胸分别为5例、4例,咯血分别为2例、1例,针道出血分别为4例、3例、胸腔积液分别为1例、2例,差异无统计学意义。结论 CT引导下经皮穿刺MWA与CRA治疗ⅠA期周围型肺癌均显示出良好效果和安全性。两种消融技术的局部控制率、PFS率及并发症发生率比较差异均无统计学意义,均可作为早期肺癌治疗手段。
