摘要:
目的 研究C反应蛋白-甘油三酯葡萄糖指数(C-reactive protein-triglyceride-glucose index,CTI)与经皮冠状动脉介入治疗(PCI)术后支架内再狭窄(in-stent restenosis,ISR)的相关性及其预测价值。方法 该研究为回顾性病例对照研究,选择2016年1月至 2024年6月在宜兴市人民医院心内科住院的PCI术后患者492例,所有纳入患者均行冠状动脉造影(coronary angiography,CAG)确定是否存在ISR,收集临床资料,CAG术前检测血常规、血生化、随机血糖、糖化血红蛋白等。根据CAG结果分组,即非ISR组(427例)和ISR组(65例)。采用限制性立方条样(RCS)分析CTI与ISR的非线性关系; 采用logistic回归分析ISR的相关危险因素; 采用受试者工作曲线(ROC)分析相关指标对ISR的预测效能。结果 CTI水平在ISR组明显升高; RCS分析显示,CTI与ISR不呈现非线性关系(P=0.304),曲线接近线性相关; 调整年龄、性别、高血压史、糖尿病史、吸烟史后,仍然呈现线性相关(P=0.307)。单因素logistic回归分析提示CTI是ISR的危险因素(OR=2.85,P<0.05); 多因素logistic回归校正性别、年龄、吸烟、饮酒、高血压、糖尿病后,CTI仍然是ISR的危险因素(OR=1.82,P<0.05)。ROC分析结果显示,CTI ROC曲线下面积为0.71(95%CI:0.64~0.77,P<0.001); 通过计算约登指数,CTI的ROC曲线上最佳截断点为8.86,对应的灵敏度和特异度分别为87.7%、47.1%。结论 术前CTI升高与PCI术后ISR的发生存在相关性,且高水平CTI是PCI术后发生ISR的独立危险因素。
