摘要:
目的 探讨导航机器人辅助下CT引导胰腺穿刺活检的应用效果。方法 选取2025年9月至2026年2月在上海交通大学医学院附属仁济医院(东院)行CT引导下胰腺穿刺活检术的患者80例,采用随机抽样法将其分为机器人实验组40例(采用导航机器人辅助下CT引导胰腺穿刺活检),徒手对照组40例(采用传统CT引导下人工操作胰腺穿刺活检)。对比两组患者的穿刺效率、穿刺并发症、疼痛程度与穿刺体验。结果 两组手术总时间差异无统计学意义(P>0.05),机器人实验组调针次数、CT扫描次数均少于徒手对照组(均P<0.05),穿刺时间低于徒手对照组(t=7.139,P<0.001),首次进针成功率高于徒手对照组(χ2=4.588,P=0.032),患者的受辐射剂量亦低于徒手对照组(t=37.919,P<0.001)。徒手对照组并发症总发生率为10.0%(4/40),机器人实验组并发症总发生率为2.5%(1/40)差异无统计学意义(χ2=0.513,P=0.474)。与徒手对照组相比,机器人实验组术后即刻、术后1 d的疼痛评分均显著降低(t=5.057、2.211,P<0.001、P=0.030); 机器人实验组总满意度为100%(40/40),高于徒手对照组82.50%(33/40),差异有统计学意义(χ2=5.636,P=0.018)。结论导航机器人辅助穿刺系统能够显著提升胰腺穿刺活检的精准性,且安全性较高,极大改善患者的穿刺体验。
