摘要:
近距离治疗作为肿瘤局部治疗的重要手段,通过将放射源(如碘-125)直接植入肿瘤或邻近腔道,实现精准的内照射放疗。目前永久性粒子植入技术临床应用广泛,其显著提升了实体瘤的局部控制率。然而,针对腔道内肿瘤的治疗仍面临挑战:传统永久性植入易导致粒子移位,且剂量分布难以动态调控,可能影响疗效并增加并发症风险。为此,研究者创新性地设计了放射性粒子链。放射性粒子链是通过将碘-125粒子以线性的方式依次排列后,放置到导管中近距离治疗腔道内的病变,克服了永久性粒子植入穿刺难和粒子游走、剂量分布不均等问题。该文综述碘-125放射性粒子链的历史及变迁、腔内粒子链放疗的效果、粒子链植入的方法和设备,以及粒子链剂量学相关的进展,旨在为该技术在肿瘤精准治疗领域的规范化、个体化发展,为改善腔道肿瘤患者预后提供更多治疗选择。
