摘要:
目的 探讨非瓣膜性心房颤动(non-valvular atrial fibrillation,NVAF)患者全身炎症反应指数(systemic inflammation response index,SIRI)和全身免疫炎症指数(systemic immune-inflammation index,SII)与左心耳封堵术(left atrial appendage closure,LAAC)预后的相关性。方法 回顾性纳入2019年至2023年在苏北人民医院接受LAAC的254例NVAF患者。根据LAAC后是否发生主要心脑血管不良事件(major adverse cardiovascular and cerebrovascular events,MACCE),将患者分为MACCE组(n=52)和非MACCE组(n=202)。Cox比例风险回归模型分析发生MACCE危险因素。结果 两组间年龄、2型糖尿病、抗血小板药、白细胞计数、中性粒细胞计数、单核细胞计数、纤维蛋白原、SII、SIRI比较差异均有统计学意义。Cox多因素回归分析结果显示,2型糖尿病史、年龄、SII和SIRI是NVAF患者行LAAC后发生MACCE的独立危险因素。绘制受试者工作特征曲线显示,SII、SIRI及两者联合预测NVAF患者行LAAC后发生MACCEs的曲线下面积分别为0.694、0.717、0.731。Kaplan-Meier生存曲线显示,SII、SIRI及两组联合值越高,发生MACCEs风险越高。结论 SIRI、SII及两者联合与NVAF患者行LAAC后发生MACCEs有显著相关性,具有一定的预测价值,值得临床推广应用。
