【摘要】目的探究超声多模态诊断模式联合超声精准介入技术在乳腺实性肿物良恶性性质鉴别中的应用。方法纳入2021年5月至2023年7月于武威肿瘤医院行乳腺手术患者396例。所有患者均于术前2周内行超声多模态检查(二维超声、彩色多普勒超声、弹性超声、乳腺三维超声及超声造影)及超声穿刺活检。分析超声多模态检查、超声穿刺检查单一及联合诊断结果与术后病理诊断结果的一致性。结果396例患者中,237例(59.85%)为恶性病灶,159例(40.15%)为良性病灶。多模态超声诊断乳腺实性肿物良恶性的灵敏度为77.64%(184/237),特异度为90.57%(144/159),阳性预测值为92.46%(184/199),阴性预测值为73.10%(144/197),准确率为82.83%(328/396),与术后病理诊断结果的一致性为0.656。超声穿刺诊断乳腺实性肿物良恶性的灵敏度为94.51%(224/237),特异度为100.00%(159/159),阳性预测值为100.00%(224/224),阴性预测值为92.44%(159/172),准确率为96.72%(383/396),与术后病理诊断结果的一致性为0.933。多模态超声联合超声穿刺诊断乳腺实性肿物性质的灵敏度为100.00%(228/228),特异度为94.64%(159/168),阳性预测值为96.20%(228/237),阴性预测值为100.00%(159/159),准确率为97.73%(387/396),与术后病理诊断结果的一致性为0.937。结论超声多模态诊断模式与超声精准介入技术可用于辅助诊断乳腺实性肿物的良恶性,且诊断准确率较高。
