摘要:
目的 建立基于列线图的肝癌介入术后早期恶心呕吐预测模型。方法 纳入2020年2月至2023年2月在南通大学附属医院接受经导管动脉化疗栓塞术(TACE)治疗的肝癌患者247例作为建模组,根据术后早期恶心伴呕吐发生情况分为发生组和未发生组。选取2023年3月至2025年3月173例肝癌患者作为外部验证组。分析影响发生恶心呕吐的因素,构建列线图预测模型,对列线图模型进行验证。结果 建模组患者中,术后早期恶心呕吐患者76例(30.77%),发生组和未发生组患者的性别、饮酒史、肿瘤数量、晕动症史、化疗药、栓塞剂、手术时间、碱性磷酸酶(ALP)等比较,差异均有统计学意义(P<0.05)。多因素logistic回归模型分析显示,PLR、ALP、性别、肿瘤数量、晕动症史、化疗药、栓塞剂是肝癌介入术后早期恶心呕吐的影响因素(P<0.05)。ROC曲线分析显示,列线图模型的曲线下面积为0.871,敏感度为0.848,特异度为0.876。对列线图模型内外部验证显示,模型校准曲线与理想线较为一致,决策曲线分析显示,当高风险阈值为0~1.0时,净收益率>0,且风险阈值越小,净收益率越大。结论 基于PLR、ALP、性别、肿瘤数量、晕动症史、化疗药、栓塞剂构建的列线图模型对肝癌介入术后早期恶心呕吐具有较高的预测价值。
