摘要:
目的 评价桡动脉入路(transradial approach,TRA)脑血管介入治疗患者术前桡动脉超声检查的必要性和临床效果。方法 选取2023年9月至2024年9月在东部战区总医院接受TRA脑血管介入术531例患者。随机分为对照组(术前未行桡动脉超声检查)260例,试验组(术前行桡动脉超声检查)271例。比较两组间穿刺成功率、并发症发生率和患者满意度。结果 试验组首次穿刺成功率显著高于对照组(94.46%对87.69%,P=0.006),并发症发生率更低(9.23%比25.00%,P<0.001),患者满意度评分更高[(4.79±0.41)比(4.42±0.63),P<0.001]。多因素logistic回归分析和列线图显示,术前桡动脉超声检查可有效提高桡动脉穿刺成功率,同时降低术后并发症发生率。结论 术前桡动脉超声检查为介入医师提供了直观的桡动脉解剖信息和血流动力学参数并进行标记,可提高穿刺成功率,降低并发症发生率,提高患者满意度。这种优化的临床结局使患者获益。
