摘要:
目的 探讨颈动脉专用光学相干断层扫描(optical coherence tomography,OCT)在动脉瘤血流导向装置(flow diverter,FD)植入动物模型中对支架贴壁和内皮化评估的应用价值,并分析贴壁不良(incomplete stent apposition,ISA)对支架内皮化的影响。方法 建立巴马小型猪颈总动脉侧壁动脉瘤模型。FD植入前和植入后分别使用颈动脉专用OCT(实验组)和冠脉OCT(对照组)对FD植入动脉和支架内腔进行扫描,并使用各自工作站依次获得对比度最优条件下后处理图像。以OCT对实验猪血管外膜弹力层显影清晰度和完整性程度为评判标准分析比较两组图像质量差异; 分别记录实验猪不同OCT条件ISA发生数目、部位和程度,并比较二者在FD贴壁评估中的差异性。术后第12周末行数字减影血管造影(digital subtraction angiography,DSA)评估FD内血流通畅情况; 行OCT观察ISA的转归,并与组织病理学检查对比,分析ISA对FD内皮化影响。结果 共建立10头小型猪颈总动脉侧壁动脉瘤Tubridge FD植入模型,并顺利实施两种不同类型的OCT检查。相对于对照组,实验组在展示实验猪血管外膜弹力层方面具有更高的清晰度和完整性,图像质量明显提高(χ2值分别为12.69和45.68,P值均<0.001)。FD植入后,两种OCT分别监测出23处ISA[实验组,包括FD头尾端12处和FD局部11处,平均距离(492.76±46.74)μm]和21处ISA[对照组,包括FD头尾端10处和FD局部11处,平均距离(490.16±46.53)μm],差异无统计学意义(P>0.05)。术后第12周末DSA显示2枚FD闭塞和2枚FD内轻度狭窄,余6枚FD内血流通畅。除2枚闭塞FD外,OCT提示FD内大部分急性ISA消失,残留未贴壁的比例降至15.8%(3/19)。残留的ISA中1处位于瘤颈区,2处位于FD局部,组织病理学显示FD编织丝裸露,未见明显血管内皮细胞覆盖; 2枚闭塞的FD其腔内闭塞,组织病理学提示主要由平滑肌细胞和脂质成分构成。结论 相对于冠脉OCT,颈动脉专用OCT在动脉瘤FD植入模型中能显著提高腔内成像图像质量,同时OCT提示大部分ISA在12周末的随访中有较好的转归,而严重的ISA与FD内皮化不良密切相关。
