摘要:
目的 分析剪切波弹性成像(shear wave elastography,SWE)技术在冈上肌腱损伤经富血小板血浆(platelet-rich plasma,PRP)治疗后评估中的价值。方法 选取武汉市中医医院2022年6月至2025年1月期间冈上肌腱损伤并接受单次超声引导下PRP注射治疗的患者112例,分别于治疗前后进行视觉模拟评分法(visual analogue scale,VAS)、Constant-Murley肩关节功能(Constant-Murley shoulder function,CMS)评分、美国肩肘外科医师协会(American Shoulder and Elbow Surgeons,ASES)评分以及常规超声(肌腱厚度)、SWE多参数[杨氏模量(E-mean)、剪切波速度(SWV)、弹性比值(SR)]检测,比较治疗前后各指标变化,采用Pearson相关性分析探讨治疗后6个月SWE多参数变化值与临床评分变化值的相关性。结果 PRP治疗后1、3、6个月,患者VAS评分均较治疗前下降(P<0.05),CMS、ASES评分及E-mean、SWV、SR均较治疗前升高(P<0.05); Pearson相关性分析显示,ΔE-mean、ΔSWV、ΔSR与ΔVAS均呈负相关(P<0.05),与ΔASES、ΔCMS均呈正相关(P<0.05)。结论 PRP注射治疗可有效减轻冈上肌腱损伤患者的短期疼痛,改善肌腱功能状态,并促进肌腱形态学改善。基于SWE技术检测E-mean、SWV和SR可定量评估治疗后肌腱生物力学特性的恢复,治疗前后参数的变化值与临床功能改善密切相关。
