摘要:
目的 探讨导向器辅助技术在困难下腔静脉滤器取出中应用的安全性和有效性,为复杂病例提供新解决方案。方法 回顾性分析2023年11月至2024年11月在广西中医药大学第一附属医院接受导向器辅助下困难下腔静脉滤器取出术的连续7例患者临床资料。术前通过腹部CT评估滤器倾斜方向及周围解剖特征。术中经右股静脉置入导向器(RUPS-100/TPS01),调整其弯曲角度以支撑滤器伞顶,并旋转导向器使回收钩游离下腔静脉壁,联合鹅颈式抓捕器及8 F MPA Guiding导管完成滤器回收。术后行下腔静脉造影评估并发症。结果 7例患者滤器均成功取出,成功率7/7。中位手术时间58.6 min(15~180 min),中位X线曝光时间38.8 min(6.5~129.3 min)。5例需调整导向器弯曲角度,4例联合8 F MPA Guiding导管增强抓捕指向性。术中3例患者出现轻度腹痛,滤器取出后自行缓解; 未发生滤器断裂、下腔静脉狭窄或撕裂等严重并发症。结论 导向器辅助技术通过精准调整滤器倾斜角度,显著提高了困难下腔静脉滤器取出成功率,具有良好安全性。但其临床应用仍需更大样本量研究进一步验证。
