摘要:
目的 探讨CT引导下射频消融(radiofrequency ablation,RFA)联合腹腔神经丛射频热凝(celiac plexus radiofrequency thermocoagulation,CPRF)在伴有中重度顽固性疼痛中晚期原发性肝癌疼痛患者中的疗效。方法 回顾性分析2019年4月至2021年5月在南京医科大学第三附属医院接受治疗的原发性肝癌患者102例,根据治疗方案分为观察组54例和对照组48例。对照组实施RFA治疗,观察组实施RFA联合CPRF治疗。比较两组穿刺引导时间、操作时间、术后并发症、肿瘤标志物[癌胚抗原(carcinoembryonic antigen,CEA)、甲胎蛋白(alpha-fetoprotein,AFP)、糖类抗原125(carbohydrate antigen 125,CA125)]、肝功能指标[谷草转氨酶(aspartate aminotransferase,AST)、谷丙转氨酶(alanine aminotransferase,ALT)]、疼痛反应、局部复发率、总生存期(OS)和无进展生存期(PFS)。结果 两组患者的手术成功率均为100.00%,术中、术后未见严重并发症。术中,观察组和对照组的穿刺引导时间分别为31.61(26.95~38.45)min、23.07(20.24~29.17)min(P<0.05); 操作时间分别为54.59(49.47~65.26)min、41.16(31.89~50.71)min(P<0.05)。观察组术后恶心/呕吐、腹泻及腰部区域麻木的占比高于对照组(P<0.05)。治疗后,两组血清CEA、AFP、CA125、AST、ALT水平均降低(P<0.05),组间比较差异无统计学意义(P>0.05)。术后24、48 h,两组患者的疼痛评分较术前降低,观察组低于对照组(P<0.05)。术后1、3年,观察组的局部复发率分别为31.48%、53.70%,对照组的局部复发率分别为35.42%、60.42%; 两组患者均未达到中位OS,中位PFS分别为35、28个月,两组局部复发率及PFS比较差异均无统计学意义(P>0.05)。结论 RFA联合CPRF治疗能够有效减轻原发性肝癌患者术后的疼痛程度。
