摘要:
局部微创消融日益广泛地应用于实体肿瘤治疗,尤其是为无法外科手术或手术耐受性差患者提供了重要的替代方案。冷冻消融(cryoablation,CRA)作为一种基于超低温微创治疗方式,通过冷冻探针在靶区诱导肿瘤细胞坏死并释放肿瘤相关抗原,理论上能激活后续的抗肿瘤免疫反应。CRA技术潜在优势在于影像下消融边界清晰、创伤较小、痛感较低以及免疫激活较强等,已逐渐成为前列腺癌、肾癌、肺癌、肝肿瘤等多种肿瘤的重要治疗手段。该文综合介绍CRA技术作用机制、优势及其在多种肿瘤治疗中应用,重点探讨其在肝肿瘤治疗中的安全性和有效性,并总结CRA联合化疗、靶向治疗、免疫治疗等应用策略。尽管CRA展现出良好的临床应用潜能,但仍存在穿刺道出血、治疗时间较长及冷冻休克风险等局限性。未来通过设备创新、手术流程优化和大规模临床研究,CRA有望在实体肿瘤治疗中发挥更大作用,为患者提供更精准、安全的治疗选择。
