摘要:
【摘要】 目的 观察经皮穿刺不可逆电穿孔消融(IRE)联合自然杀伤细胞(NK细胞)治疗胰腺癌的安全性和近期临床疗效。方法 2016年5月至2017年12月,纳入50例胰腺癌患者,随机分配至两组。一组为IRE治疗(IRE组,25例),另一组为IRE联合NK细胞治疗(IRE- NK组,25例)。观察患者免疫功能变化,CA19- 9变化和CT检查结果来评估胰腺癌的治疗效果。结果 所有患者均安全完成IRE手术及NK细胞回输治疗。所有患者治疗后的淋巴细胞计数及淋巴细胞功能均较术前显著增高,差异有统计学意义(P<0.05);所有患者治疗后60 d时的CA19- 9水平较手术前显著下降,差异有统计学意义(P<0.05),IRE- NK组的CA19- 9下降程度较IRE组更明显。所有患者治疗后60 d时CT评估结果显示肿瘤直径及CT值均较术前下降。IRE组的肿瘤缓解率是56.0%, IRE- NK组的肿瘤缓解率是68.0%。结论 IRE消融联合NK细胞治疗胰腺癌安全可靠,两者具有协同治疗效果,表现在它们不仅增强患者免疫功能,同时减少CA19- 9的表达,缩小肿瘤直径及CT值,提高肿瘤缓解率。
