【摘要】 目的 探讨建立稳定的门静脉主干癌栓(MPVTT)动物模型并作客观评估,为进一步临床治疗研究提供基础。方法 24只新西兰大白兔随机分成对照组(A组,n=10)和实验组(B组,n=14)。实验组在门静脉主干前壁缝一荷包,将瘤条经荷包中央注入门静脉腔内,并通过预留缝线悬挂固定在门静脉主干内壁;对照组开腹后仅在门静脉主干前壁作荷包缝合。术后观察动物一般情况、体重及生存时间。术后每周行多排螺旋CT检查,观察门静脉癌栓(PVTT)生长及转移。分阶段处死两组部分实验兔作病理检查,观察PVTT体积和转移情况。剩余兔观察生存时间。结果 实验组门静脉主干成瘤率达100%。实验组(B9~14号兔)及对照组术后第35天平均体重分别为(1.48±0.19) kg和(2.08±0.17) kg,实验组(B9~14号兔)平均生存时间为(41.7±4.72) d。多排螺旋CT显示PVTT及门静脉阻塞后侧支循环和转移灶,病理学检查证实门静脉管腔内癌栓存在及转移灶。结论 成功建立了稳定的可供影像学客观评价的MPVTT动物模型,并证实增强多排螺旋CT对诊断和监测PVTT生长和转移有良好价值,为临床治疗MPVTT研究建立基础。
