 【摘要】 目的 探讨320排动态容积CT血管造影（320排CTA）对颅内动脉瘤手术前、后评估的价值。方法 2010年4月至2012年10月经320排CTA检出动脉瘤患者90例，采用敏感度、特异度、阳性预测值、阴性预测值评价320排CTA对颅内动脉瘤的诊断价值。对60例患者（20例行血管内介入栓塞治疗，40例行开颅动脉瘤夹闭）术后进行320排CTA随访，对比手术前、后CTA图像，评价手术效果。结果 90例阳性患者中共检出100枚动脉瘤，其中 ＜ 3 mm的28枚，≥ 3 mm的72枚。320排CTA诊断3 mm以内动脉瘤的敏感度为96.4%，特异度为100%，阳性预测值为100%，阴性预测值为90.9%，对≥ 3 mm动脉瘤的敏感度、特异度、阳性预测值和阴性预测值均为100%。60例患者术后未发现动脉瘤残余，载瘤动脉均未见狭窄或闭塞。结论 320排CT颅脑一站式检查是一种快速、易行和可靠的检查方法，可作为颅内动脉瘤筛查、术前评估和术后随访的首选技术，但可能遗漏微小动脉瘤，3D?蛳 DSA仍是诊断颅内动脉瘤的金标准。
