 【摘要】 目的 通过对活体猪股骨、胫骨骨端和骨干进行射频消融术(radiofrequency ablation,RFA),观察骨端和骨干损毁后的修复情况,探讨活体动物长骨损毁后RFA灶的修复情况,为骨肿瘤的临床RFA治疗提供依据。方法 取云南小耳猪6头,在麻醉下,于不同时间分次对股骨、胫骨骨干、骨端行RFA,并于RFA后即刻、3、10、24 d和5、8、12周处死动物,取骨干、骨端标本,进行骨切片和HE染色,镜下观察RFA灶组织学改变。同时采用X线和CT检查3、10、24 d和5、8、12周骨干、骨端RFA后的骨质修复情况。结果 RFA后3 ~ 10 d,骨质RFA边缘充血出血带内见炎性反应及肉芽组织形成;RFA后10 d, RFA灶边缘可见低密度的骨质吸收带;RFA后24 d可见RFA灶外缘最先形成的纤维组织内有新生骨质形成,X线和CT检查见RFA灶边缘的骨质吸收带更宽,而且RFA灶骨质吸收带外缘的骨质增生硬化;5、8、12周时,RFA灶修复性骨质增生更明显,骨端RFA灶低密度骨质吸收区缩小,骨干髓腔因骨质增生而狭窄。RFA灶邻近的骨外膜可有反应性成骨,形成骨膜下条状骨痂。结论 RFA后3 ~ 10 d,在RFA灶边缘的出血充血带可见炎性反应及肉芽组织形成,10 d时表现纤维性修复,24 d时表现骨性修复。
