 【摘要】 目的 研究在血管内超声(IVUS)检测到的破裂斑块部位行冠状动脉介入治疗(PCI)后冠状动脉的血流变化。方法 选择2007年6月至2011年5月接受择期冠状动脉造影(CAG) 并行PCI的冠心病患者36例,按IVUS所见,将患者分为有破裂斑块(RP)组18例和无破裂斑块(CTL)组18例,应用校正的TIMI帧数法(cTFC),研究PCI术后冠状动脉的血流变化。结果 RP组共发现18处破裂斑块,破裂斑块空腔面积平均为(3.3 ± 2.8)m2,破裂斑块处斑块负荷平均为68.7% ± 12.1%,CTL组为69.2% ± 11.7%。RP组平均cTFC由术前25.5 ± 8.0降为22.1 ± 5.3(P = 0.13),CTL组则由术前25.2 ± 7.6下降到15.1 ± 4.0,组间差异有统计学意义(P < 0.01)。结论 与CTL组相比,在破裂斑块处行PCI后冠状动脉血流TIMI帧数下降不明显,可能与该处介入后冠状动脉微小血管功能障碍有关。
