【摘要】 目的?研究吉非替尼联合125I放射性粒子治疗肺癌A549细胞裸鼠移植瘤有效性研究。方法?利用稳定表达荧光素酶的A549-luc人肺腺癌细胞建立裸鼠皮下移植瘤模型24只,20 d后成瘤大小8~10 mm。24只荷瘤裸鼠随机分为空白对照组6只、125I放射性粒子植入组6只,吉非替尼组6只,吉非替尼联合125I放射性粒子植入组6只。观察肿瘤生长情况,持续测量肿瘤大小变化。利用活体生物发光成像检测含荧光素酶报告基因的人肺腺癌A549-luc细胞在体内的生物发光活性。治疗前和治疗后1周行18F-FDG Micro-PET/CT检查。35 d后处死裸鼠,绘制肿瘤生长曲线。结果?4组治疗前肿瘤体积差异无统计学意义,治疗后5周4组(对照组,125I放射性粒子组,吉非替尼组,联合用药组)肿瘤体积分别为(1 509.25±709.93)、(840.45±43.35)、(1 052.96±247.42)和(317.34±52.08)mm3,联合组与另外3组比较差异有统计学意义(F=10.305,P<0.05)。吉非替尼联合125I放射性粒子明显低于吉非替尼组、125I放射性粒子组和对照组,且明显低于治疗前;4组治疗前肿瘤生物荧光信号强度无明显差异,治疗后4周4组肿瘤生物荧光光子数分别为(198 605 000.0±12 976 503.00)、(99 263 333.33±49 293 480.57)、(87 419 500.0±24 039 740.21)、(48 433 333.33±14 417 910.62)P/sec/cm2/sr,差异有统计学意义(F=28.975,P<0.05),吉非替尼联合125I放射性粒子植入组明显低于单独125I放射性粒子组、吉非替尼组和对照组,且明显低于治疗前。4组治疗前SUVmax和SUVmean值差异无统计学意义,治疗后1周4组SUVmax和SUVmean值分别为0.79±0.14和0.54±0.06、0.76±0.33和0.47±0.41、0.79±0.15和0.48±0.11、0.74±0.14和0.57±0.74,差异无统计学意义(F=0.201, P>0.05)。结论?吉非替尼联合125I粒子近距离治疗能显著抑制肿瘤的生长。
