【摘要】 目的 探讨无水乙醇和泡沫硬化剂治疗儿童四肢静脉畸形(VM)的安全性和有效性。方法回顾性分析21例四肢VM患儿,根据Puig分型采用单纯无水乙醇/泡沫硬化剂治疗或联合应用治疗,随访观察疗效、不良反应和并发症。结果 21例患儿共治疗32次(1~3次/例),均达到症状或功能改善,有效率为100%。术中无水乙醇平均单次应用剂量为(0.21±0.12) mL/kg,3%聚多卡醇泡沫硬化剂平均单次应用剂量为(5.10±2.70) mL。术后随访6~14个月,患儿加拿大东安大略儿童医院疼痛量表(CHEOPS)评分均值(4.51±2.32)较术前(10.25±2.44)明显下降,表明患儿症状明显改善;无明显症状复发、不良反应和严重并发症。结论 根据Puig分型选择无水乙醇、泡沫硬化剂或两者联合治疗四肢VM病灶安全有效,尤其是对部分病灶联合应用,可降低无水乙醇应用量,进一步提高治疗安全性和疗效。
