【摘要】 目的 对比肝动脉化疗栓塞(TACE)联合125I粒子植入与单纯TACE治疗原发性肝癌(PHC)合并门静脉癌栓(PVTT)的临床疗效及安全性。方法 计算机和人工检索PubMed、Cochrane图书馆、CBM、万方和中国知网等数据库收录的关于比较TACE联合125I粒子植入与单纯TACE治疗PHC伴PVTT临床疗效及并发症的回顾性队列研究,研究者从中提取相关资料,根据Cochrane手册标准对纳入文献的质量予以评价。结果 最终8篇文献纳入研究,病例总数为822例。结果显示,TACE联合125I粒子植入组治疗肿瘤病灶及PVTT有效率、控制率高于单纯TACE组,差异均有统计学意义(P<0.05);TACE联合125I粒子植入组0.5、1、2年生存率优于单纯TACE组,差异均有统计学意义(P<0.05);然而在严重并发症发生率方面两组差异无统计学意义(P>0.05)。结论 TACE联合125I粒子植入治疗PHC伴PVTT的临床疗效优于单纯TACE,两组的严重并发症发生率无明显差异。
