【摘要】 目的 观察静脉溶栓桥接机械取栓治疗急性脑梗死患者的临床效果和安全性。方法 回顾性分析2015年9月至2018年3月间收治的急性颅内前循环大动脉梗死患者70例,根据不同治疗方法分为单纯静脉溶栓组(n=37)、静脉溶栓桥接支架取栓组(n=33)。对比两组患者治疗后血管再通率、24 h美国国立卫生院卒中量表(NIHSS)评分改善率、90 d改良Rankin 量表(mRS)评分改善情况、脑出血和30 d病死率。 结果 两组治疗前NIHSS评分、mRS评分差异无统计学意义(P>0.05)。治疗后溶栓桥接支架取栓组血管再通率显著高于单纯溶栓组(P<0.01),24 h、14 d NIHSS评分均低于单纯溶栓组(P<0.05),90 d mRS评分低于单纯溶栓组(P<0.05)。两组治疗后30 d内颅内出血率、90 d病死率差异均无统计学意义(P>0.05)。 结论 静脉溶栓桥接支架取栓治疗急性期脑梗死伴大动脉狭窄患者安全有效,血管再通率、90 d mRS评分和预后优于单纯静脉溶栓。
