【摘要】 目的 探讨药物洗脱球囊(DEB)对糖尿病伴分叉病变、小血管病变、支架内再狭窄(ISR)病变患者经皮冠状动脉介入治疗(PCI)预后的影响。 方法 入选2016年6月至2017年6月119例冠状动脉造影证实为分叉病变、小血管病变、ISR病变患者,根据病变情况随机予以DEB治疗(n=60)或药物洗脱支架(DES)治疗(n=59)。术后12个月复查靶病变冠状动脉造影。观察两组患者术前、术后即刻、术后12个月靶病变最小管腔直径(MLD)及术后12个月主要心血管不良事件(MACE)。 结果 两组患者术后即刻MLD较术前均显著提高,DEB组术后即刻MLD(2.64±0.07) mm小于DES组(2.83±0.05) mm(P=0.032),术后12个月差异[(2.75±0.03) mm对(2.78±0.08) mm]无统计学意义(P=0.353)。两组患者术后6、12个月 MACE发生率差异无统计学意义(P>0.05),无MACE生存Kaplan Meier曲线具可比性(P=0.552)。 结论 DEB治疗糖尿病伴分叉病变、小血管病变、ISR病变安全可行,疗效不低于DES。
